Record Retrieval Services

We obtain medical records, business records, and payroll and personnel records by deposition on written questions. We obtain records from state and federal agencies such as the Department of Workers Compensation or Social Security Administration. We also obtain certified copies of police records.

Whether you need medical records, x-rays, business records, personnel files, payroll records, or police records, Blend Litigation Support understands how to work with records custodians and how to negotiate the bureaucratic maze of dealing with state and federal agencies, like the Social Security Administration.

You may fill out our order form and fax it to our office, or you may provide the requested information via email.  If you have an authorization in your possession, please forward that as well.  If we use the authorization, you will be informed of this fact.

Within twenty-four hours of receiving your order, we send a request by authorization to the records custodian or, in the case of deposition on written questions, send out the notice of intention. For requests by subpoena and written deposition, we understand that time is of the essence.  We will contact the opposing counsel within three days to seek waiver of the 14-day (Federal) or 21-day (State) waiting periods.  We attempt to begin gathering records immediately.

Please be assured that our case managers work persistently to ensure that records are produced timely and that all required rules are followed.  We will follow up with slow-moving custodians and keep you informed with regard to record status.  Status reports are available online and can be forwarded automatically upon request.

Records are delivered bound, bates-labeled, with DWQ completed and affidavits signed and notarized. DWQ records are accompanied by a sealed court copy. Digital records are delivered by e-mail or on CD, or we can house your digital records in our online file repository.
